Pediatric Symptoms Checklist

The Pediatric Symptom Checklist is a psychosocial screen designed to facilitate the recognition of cognitive, emotional, and behavioral problems so that appropriate interventions can be initiated as early as possible.  Not only can this screening identify issues such as anxiety, attention, depression and anger, but it also acts as a tool to help connect family with resources.  Feel free to complete this screening tool on your child and submit to the WBHN Early Recognition Specialist who will evaluate the results and contact you to discuss resources and help develop a plan.
